About Us


Preferred Health Care (PHC) is a Medicare Certified Home Health Agency licensed in the state of Illinois. PHC is located in Itasca. PHC currently services 16 counties.

We have the knowledge and expertise to assist patients and their families with their health care needs. Preferred Health Care specializes in disease and wound care management, Alzheimer’s, and dementia. PHC has a wound care healing rate of 83%, above the state and national average. We are Medicare approved for home psychiatric services. Daily visits are given to our patients if medically necessary.

The provision of home health services by PHC is guided by the mission and philosophy. This mission is to provide for the effective and efficient delivery of health care and health-related services in areas of identifiable need, for the benefit of individuals, families and society. PHC is committed to maintain a Christian emphasis in all its endeavors.


PHC is dedicated to providing, directly or through contractual agreement, high quality nursing, physical therapy, occupational therapy, speech therapy, social work services and home health aid services to individuals of all ages regardless of national origin, race, religion, sex, handicaps, and financial status.



PHC has grown through the years. Preferred Health Care initially serviced only 3 counties and has expanded regionally to service 16 counties.

Preferred Health Care is an Illinois Medicare Certified Home Health Agency with an experienced multilingual field staff; available 24 hours a day, 7 days a week.


PHC has the knowledge and expertise to assist our patients and their families with their health care needs. We can provide an admitting nurse the same day as a hospital discharge; with supplemental services if necessary such as therapy, certified nursing aides, social workers, home medical equipment, and wound care.
